AGE UK HILLINGDON
Established in 1989 we are an independent charity working solely in the London Borough of Hillingdon and are a member of the Age UK network. We aim to promote independence and wellbeing for people in later life through our wide range of services. Our goal is for “every older person in Hillingdon to have someone to turn to”. Our services include: Information & Advice - including benefits advice, housing and care, will writing and advance decisions. Help at Home – handyperson, cleaning, shopping, accompanying to appointments. GP and Hospital Support – A&E support staff, home from hospital, care navigation, falls prevention, nail cutting. Social Wellbeing – befriending, a range of groups and clubs, mental health support groups, living well with dementia, exercise groups, healthy living social clubs.

Age UK Horsham District
Age UK Horsham District is a local charity established in 1958. We work across the Horsham District to provide services and activities that promote, influence and improve the wellbeing and quality of later life in Horsham District. We are brand partners with Age UK and have many excellent working partnerships with organisations including Horsham District Council, West Sussex County Council, Horsham District Older People’s Forum and Age UK West Sussex; all these partnerships enable us to provide an extensive variety of services.

Age UK Hythe and Lyminge
We work to improve later life by providing a range of holistic services for older people and their carers across the two areas of Hythe and lyminge in Kent. Our current services include provision of Social Opportunities/Daycare, Health and wellbeing services such as hairdressing and footcare, community services such as home meals, independent living services such as housework, gardening, visits and accompanied outings, information and advice to maximise income and much more. We have 54 volunteers involved in all areas of the organisation. We are the result of two local organisations Age Concern Hythe and Age Concern Lyminge merging in 2011 at the same time we joined Age UK as a brand Partner.

Age UK Kensington & Chelsea
Age UK Kensington & Chelsea supports older people to remain connected within the community, offering flexible services designed with the views and preferences of our members at their core. These range from befriending, escorting, practical help and homecare to intergenerational IT coaching and friendship groups based on shared interests. Our dementia work encourages people living with the condition to claim their place in our community and our information and advice team deals with enquiries ranging from benefits to support planning. We have over 350 volunteers aged 14 to 94 involved in all areas of the organisation. The principle for our way of working is that there is no wrong door. The charity today is the result of a merger in 2009 between two local organisations of more than 20 years standing - Sixty Plus and Age Concern Kensington & Chelsea.

Age UK Lancashire
Age UK Lancashire provides services for older people to live healthy and active lives and to remain independent and connected with their communities for as long as possible. Our local teams provide support to promote good physical and mental health across Lancashire this includes the main areas of population and in many of the rural areas. Our services are designed to support all older people and their carers who either live alone or with support, assisting them to retain or regain skills and maintain relationships. We support older people with disabilities and people with memory problems and people living with a diagnosis of dementia. All services and activities:- •Increase choice and control to ensure people are supported in friendly neighbourhoods; able to get out and about and have positive things to do. •Enable people to play a more active role in managing their own lives •Proactively promote good health, by focusing on support that aims to enhance well-being and to avert crises; •Adopt a whole-person approach, by ensuring our services are person centred and take account of individual wants and needs. •Ensure the best outcome for carers by providing appropriate and timely information advice and support.

Age UK Leeds is a local independent charity. Our mission is to work with and for all older people in Leeds to improve later life. Our vision is a society where older people are respected, valued and able to contribute to their community. We provide a range of services to older people in Leeds who are our beneficiaries. They include health and wellbeing activities, e.g. walking for health, digital inclusion e.g. Silver Surfers IT training, Advocacy, Information and Advice, and Social Care e.g. support to people with mental health issues, a long term condition and men with prostate cancer. Our charity is supported by the profits from 2 trading arms: one that sells insurance and other related products and goods; the other is the Are Café which operates as a social enterprise in the centre of Leeds and adjoining our offices.

Age UK Lindsey
Age UK Lindsey, covers East West and North of Lincolnshire. We offer a range of services aimed at people aged 50 and over, including befriending, help at home, benefits advice, lunch clubs, information service and coffee mornings. Age UK Mid Devon
Age UK Mid Devon works to provide information, advice, signposting and support to people with dementia and their carers. Our Information & Advice service provides practical advice and support to people following diagnosis, and helps with negotiating the health care environment. Our Advocacy team help with identifying what benefits and entitlements people can claim, and help with filling out forms and following up any appeals. Our Volunteer Visitors can help reconnect older people to their community, finding clubs or groups for older people to attend and encouraging them to make new friends and build support networks. Our Activity Day Centre at The Haven helps keep people active, mentally and physically, and provides a place where people with dementia or their carers can come and relax for a day. We take a person-centred approach to providing support and services, putting the client at the heart of what we do and ensuring that their needs are paramount.

Age UK North West Kent
A local independent charity serving the needs of older people in the Boroughs of Gravesham and Dartford and the surrounding areas. We are made up of two service divisions: Day Services which are operated from our own premises in each Borough, offering a range of well-being, health and leisure activities and interests. Within that service is our specialist ‘Day Break respite service for carers of people living with dementia : Community Services which deliver all of our outreach services, some of which are CQC regulated e.g. Assisted Bathing and ‘Simply Nails’ nail cutting service. In addition a host of other Home Support Services enabling the individual to live at home for as long as possible and maintain independence.
