Yorkshire Move Manager
At the Yorkshire Move Manager we provide specialist support for older people (55+) who wish to move home. We also provide assistance to help older clients create a safer living environment in their current home, through tackling hoarding issues and adaptations. As an experienced, trained and accredited Senior Move Manager, I offer an invaluable resource during a stressful time, supporting clients with a range of conditions including Dementia, through the entire moving home process. I am the first UK member of the National Association of Senior Move Managers. I have undertaken training on topics including dementia, memory, ethics, safeguarding and guiding clients to achieve the best in later life. Each client has different needs - I provide a package of bespoke support, delivered on a 121 basis, designed to ensure they remain in full control of their life and achieve the outcomes they desire.

Yorkshire Sculpture Park
Yorkshire Sculpture Park seeks to provide a centre of international, national and regional importance for the production, exhibition and appreciation of modern and contemporary sculpture. Many inspirational elements combine here to create a unique and exceptional balance of art, heritage, learning, space and landscape. The organisation has grown over the last 35 years: from humble beginnings with £1,000 to fund a small exhibition of 31 sculptures, to now contribute £5 million to the local economy and is responsible for five indoor galleries set in 500 acres of the 18th century-designed Bretton Estate, reunited in recent years under YSP’s stewardship. Within a national and European context YSP is unique, offering artists and visitors experiences and opportunities unlike anywhere else. ‘Great art for everyone’ has been YSP’s goal since opening to the public in 1977, enabling access, understanding and enjoyment of art and landscape for everyone.

Young at Heart Club
We provide day services for older lonely people in our community, three days a week at Uplowman and Burlescombe and established in 2014. We aim to relieve the feeling of isolation and loneliness for the over 60’s. We offer our service to residents in Mid Devon and West Somerset. The club provides somewhere for older people, of every background, to socialise with people of the same age group in safe, comfortable surroundings. Members enjoy a nutritional two course lunch and the activities are stimulating and fun which improves their well-being and self-confidence. Our club currently has 50 attendances per week, some with early on-set dementia and we are fortunate to have qualified volunteers to assist and ensure the members are well looked after throughout the day.

Younger People With Dementia (YPWD)
YPWD (Berkshire) was set up in 2012 and our aim was to provide a range of support and ancillary services to younger people with dementia (those diagnosed below the age of 65), and to their families and carers, within the Berkshire West area covering Wokingham, Reading and Newbury. We sought to provide advice, help and support to enable people affected by young onset dementia to have a fuller life, offer assistance, whether practical, motivational or through a mix of communication, personal meetings and social events, to enable them to better cope with the challenges that faced them. We have since expanded to provide our services within the whole of Berkshire.

Your Healthcare
Neurodevelopmental Services is a specialist multi-disciplinary team which is part of Yourhealthcare CIC. We provide specialist health and psychological services to people affected by neurodevelopmental conditions, including people with learning disabilities. We have a long-standing dementia care pathway within NdS which supports people with learning disabilities and query/diagnosed dementia across the boroughs of Kingston and Richmond. Our dementia care pathway runs from the points of awareness training to end of life and all members of the multi-disciplinary team may be involved at some point in the care pathway.

Your Living Room CIC
We deliver a person centred approach to people living in hoarded or cluttered environments. We currently work throughout Essex. We work wih partner organisations such as the Safeguarding Board, NHS, Social Services, Mental Health, Police, Fire service, statutory and voluntary support agencies. All of our staff are either Dementia friends or Champions. Our service goes beyond the work required to de-clutter the home by also delivering after care and support for our clients so that they do not fall back into chaos. I am the current Chairman of the Pan essex dementia action alliance and can see the value in working in partnership with organisations where there is an identified crossover of service provision. A number of the clients we are currently engaged with are living with dementia, so some of our assistuve technology aids will help alleviate their frustrations, enabling them to live well in their own homes, engaging with relatives and friends whilst supporting their safety.
