Wimbledon Civic Theatre Trust
Wimbledon Community Theatre Trust (WCTT) facilitates exciting and innovative theatre-based education projects for young people aged 5-19 in Merton, South London. Our mission is to improve access to the performing arts for children and young people, with particular emphasis on those who are hard to reach or who experience some form of disadvantage, whether that be through social or cultural exclusion, economic hardship, disability or simply the lack of opportunity to make friends and socialise. We run a range of theatre and arts-based projects in partnership with schools and community groups which engage young people who would not otherwise have the opportunity to access and experience the performing arts and in the last 12 months we have worked with over 4000 young people. All of our projects are undertaken by skilled, qualified professionals, the outcomes carefully measured and the results fully evaluated. WCTT runs a year-round Young Actors Company based at New Wimbledon Theatre Studio which is free for all young people who live or attend school in Merton.

Wimborne Salvation Army
We are a Christian Church, with Heart to God and Hand to man. We work within our community, providing a parent and toddler group, two regular coffee mornings and a weekly lunch club. We also support people in various settings, leading worship services in care homes, with a specific ministry in a dementia care home in Blandford. Within out congregation and community setting's we have people living with dementia.

Wincanton Health Centre
Dr Iain Phillips is Dementia lead for Somerset and supports his home town of Wincanton to become a dementia friendly community. Wincanton Health Centre plan to train all members of staff in dementia awareness training enabling staff to support and offer additional time to patients who may have memory problems or confusion. The practice staff will work to identify patients who may be living with dementia and offer health coach support to improve their health and wellbeing; sign posting them to support agencies to help both the patient and their carer with advice. The Practice supports the community partnership and from this the health coaches have lead the dementia friendly community steering group meetings that are hosted every 6 weeks at the practice with several members of staff attending these meetings and proactively encouraging the community to engage in the promotion of the DFC. The practice has engaged with the fire service to create a good working relationship and plan to sign an agreement to identify and act as a recognised referrer for patient home safety checks by the fire service. The practice plans to be part of the local team of recognised places of safety for patients to find help and staff will be trained in how to respond to anyone distressed or confused. The health centre are encouraging all identified patients and their carers' to carry a card identifying them as either a person living with dementia or a carer to a person living with dementia. The practice will invite carers' to any carer events held at the practice and inform them of any support groups or local training available. Two members of the steering group who live with dementia plan to visit the health centre and report recommendations on any improvements to facilities such as the signage, noise levels and telephone access that would improve their access to health. The practice have offered the premises for community dementia friend sessions and have dementia information on their waiting room television screen, social media, the web site as well as carer promotional boards and information leaflets.

Winchester City Council
Winchester City Council (WCC) is one of 11 district councils in Hampshire. Covering an area of 250 square miles it has approximately 114,300 residents, 32% of which live within the town area of Winchester. The 3 largest settlements outside the City are Bishops Waltham, Denmead and New Alresford. Part of the district straddles the M27 corridor and urban areas to the south including part of Whiteley and West of Waterlooville. The Council provides a number of services including planning, waste collection, housing, revenues and benefits, environmental health and leisure. It also works in partnership with a wide range of statutory agencies and voluntary sector organisations to deliver key outcomes identified in the Winchester District Sustainable Community Strategy. The Active Communities outcome focuses on the people that work and live in the District, encouraging healthy lifestyles and active participation in their communities and ensuring that they can access the services they need. Dementia is a key priority for WCC. In 2009 the Winchester District had the third highest number of people with a formal diagnosis of dementia in Hampshire - this number is expected to increase from 1,692 in 2012 to 1,900 in 2015, 2,200 in 2020, 2,550 in 2025 and 2,984 in 2030 - it is estimated that more than half of dementia cases are undiagnosed. Winchester will remain in the top half of districts for numbers of people with dementia for the next two decades. Dementia is also associated with a range of other long-term conditions, and has clear links to incidents of accidental fires and injuries. It also has major health and wellbeing implications for carers, many of whom are elderly themselves.

Our vision is a society in which disadvantaged people can lead fulfilling and independent lives, fully participating in society. WIRED services consist of Carers Support, Direct Payments, Managed Budgets, PALS, Payroll Service, Shopmobility, Training and the SEND Partnership. Carers Support provides access for unpaid carers to; the Carers Helpline, Carers Emergency Contact scheme, Counselling Service, Welfare Benefits Advice, Employment Support, Hospital Discharge support, Information Drop In’s referral to local services and the Carers Grant. The Carers Team are also involved in supporting Parent Carers and raising awareness of Carers issues at a local and national level.
