Age UK is here to inspire, enable and support older people to make the most of later life. The work and support we provide at Age UK covers a number of different services, including: Information and advice We provide information or advice on anything from health to housing. We also publish impartial and informative factsheets and advice guides. Campaigns Our Public Policy, Public Affairs and Campaigns teams raise awareness of the difficult situations faced by many older people and keep ageing issues in the public eye and in parliament. The issues we campaign on can vary, from calling for reform of the care system to trying to improve the bus route in our local community. Our campaigns offer lots of opportunities for us all to get involved and be heard by those who have the power to make our lives better. Commercial products Most of us want to remain independent - living in our own home for as long as possible, and keeping doing the things in life that we love the most. Our products and services are tailored to the needs of people in later life. We offer travel, motor and home insurance; funeral plans; gas and electricity packages; and aids for the home, such as stairlifts and personal alarms. Training We are a successful national training provider that reaches over 10,000 learners annually. Our training services have received recognition from Ofsted and the sector skills council Skills for Care. Our work involves the tailored provision of innovative learning solutions designed to address the needs of local people of all ages, professionals and employers throughout the UK and internationally. Specifically we offer: Apprenticeships, qualifications, short courses and skills development programmes for success at work, particularly in the social care and health sectors. Employment acquisition initiatives, ‘planning for retirement’ services and learning in later life. Specialist support for providers of services to people in later life. Research Age UK is the only UK charity to focus research funds exclusively on later life. That's because we believe that we should all be able to enjoy good health in later life, free from the diseases and disabilities associated with growing older and the consequences that they can bring about, such as social isolation, anxiety, frailty and loss of independence. The research we fund is providing answers and we've already seen significant breakthroughs in dementia, falls prevention, stroke and incontinence, to name a few.

Age UK Wolverhampton
Age UK Wolverhampton works to promote the well-being of older people and help make later life a fulfilling and enjoyable experience. Our aim is to make life better for older people. These objectives are achieved through a range of services for older people across the City including Information and Advice, Help at Home, Telephone Befriending, Digital Inclusion, and one to one support during difficult times including Home from Hospital. Airedale, Wharfedale and Craven Clinical Commissioning Group
Membership of the CCG is made up of 16 GP practices working together to plan and commission local health services for a population of 156,000. We cover two local authority areas and aim to be a leading CCG through successful integration and transformation of health and social care, the introduction of innovative, improved clinical pathways, sound financial management and a model employer, developing future clinical leaders. Our CCG has been identified as a national integrated care pioneer site. It is developing new models of care so people in the CCG’s area receive individual seamless care, reducing their need for urgent and unplanned care by proactively managing their physical, psychological and social care needs. The CCG new models of care programme is looking at new ways of working in different areas of health and social care, including the four projects below: • complex care • enhanced care • wrap around care • self -care Airedale, Wharfedale and Craven is one of 25 of the national Pioneer sites acting as an exemplars to address local barriers to delivering integrated care and support locally, and highlight national barriers that national partners can work to address. We are supporting the rapid dissemination, promotion and uptake of lessons across the country and receive support to breaking down these barriers from the national partners.

AJM Healthcare
The wheelchair service is for residents currently registered with a GP in the Plymouth, South Hams and West Devon areas. The service will provide you with all NHS wheelchair services, including clinical assessments specialist seating delivery repairs and maintenance collection when the wheelchair is no longer required We provide a free collections service. Wheelchairs are valuable, so we collect and recycle and refurbish them to an as-new condition for re-issue.

Al Hikmah Centre
Our vision is to represent the community at all levels with a mission to enhance the economic, social and spiritual, health & well being of the community in particular the community living in the IMWS catchment areas in Kirklees. We promote social cohesion / inclusion and tackle deprivation.
