1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Our project's aims are to:

- To organise/host events that promote dementia awareness

- To organise and facilitate Dementia Friends sessions, encouraging as many residents as possible to attend a session over the next 4 years

- To work with businesses, councils, housing associations and organisations to create a dementia-friendly public realm and environment.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Effective communication. How can we reach the maximum number of people across the community.
