GP Primary Choice Limited

GP Care advisors are employed by GP Primary Choice Ltd and cover all surgeries within North East Essex The Care Advisors assist and advice on the following subjects: • Benefits and completion of claim forms • Equipment Assessment of patients homes. • Occupational therapy assesments for home aids • Contact with the appropriate Statutory Care Services • Contact with the appropriate voluntary services • Identify whether patients are entitled to any form of grant • Carryout Mini Mental Health checks following the Dementia Pathway(including referral to secondary care) • See patients identified with COPD and follow COPD Pathway • See patients identified with frailty and follow Frailty Pathway • See patients that are identified as frequent users and follow Frequent Users Pathway • Support Careers and their families by sign posting and looking at benefits that are available

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

The GP Care Advisors support this cohort of patients by:

  • Listening to patients, carers and families
  • Providing advice, support, information and assist in obtaining services from other agencies and where appropriate make initial contacts on the patients behalf
  • Providing assistance to careers who are caring for a person with Dementia
  • Assisting with the completion of welfare benefit claim forms
  • Sign posting to the various groups and meeting in the local area
  • Providing assessment of needs for Telecare, equipment and services within the patients home
  • Caring out Mini Mental health checks and organising tests required by the GPs to aid the speed of first initial referral to secondary care
  • Referral to Kingswood centre

 

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

The challenge is managing staff time and the shortage when experiencing staff leave or sickness.  We acknowledge that we need to highlight those surgeries that are not at present Dementia friendly.  Staff training to become Dementia Friendly Surgeries.  Time to organise drop in afternoons at the surgeries for patients with Dementia and their careers.  Time to look at patients on dementia medication with no diagnosis of dementia recorded.

  • Make all GP practices within NE Essex Dementia Friendly Practices

    For GP Care Advisors to engage with patients and staff and take an active role with the Practice Managers in achieving Dementia Friendly Practices ensuring all staff have awareness sessions    .

  • Run searches within the clinical systems to check for patients on the list of Dementia Medication

    For a member of staff to ensure that all patients on Dementia drugs have a diagnosis of dementia, planning taking place with the new CCG Dementia Lead

  • Drop in afternoon for Dementia Patients

    To work with both GP Care Advisors and Practice Managers to develop quarterly drop in meetings within practices for both Dementia Patients and Carers.

  • For GPCA to focus on the Dementia Pathway

    Patients highlighted to the GP Care Advisors to be offered a mini mental health test at the earliest convenience and if they are found to score low the necessary test carried out and referral done if needed – following the guidelines outlined in the attached pathway.
