1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We offer all available treatment options so that patients can make an informed decision. We operate a patient centred approach to care. We support patients to live pain and dental disease free We provide patients with information leaflets and a printed treatment plan We offer a valued service to the local community. If there was an opportunity to contribute to research we would be happy to participate.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

We understand that the population is living longer and we may see more of our patients developing dementia. As patients move in to later stages of Dementia we may have to refer them onto specialised services.

  • All our staff to receive Dementia Training

    We have arranged for all our staff to have Dementia training

  • Design and implement protocol to support patients living with Dementia

    We are going to develop a protocol to establish contact details for our patients for elderly patients for a carer or next of kin, as well as a care package.

    We are going to place better signage within the practice so pateints cab better naviage around

  • Investigate looking into improving access to the patient toilets

    We are going to look into adapting the pateint toilet to make it accessible to wheelchair users
