1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?
We offer all our patients all available treatment option so that they can make an informed choice.
We offer patient centred care to meet their own specific needs.
We support our patients to live pain and dental disease free.
We offer written treatment plans and patient leaflets.
We value patients opinions and act upon reasonable feedback.
We offer a valuable service to the local community.
If there was an opportunity to contribute towards research we would be happy to participate.
2. What are the challenges to delivering these outcomes from the perspective of your organisation?
We understand the cases of patients living with Dementia will increase. When a patient moves into later stages we may have to consider referring them to specialist services for treatment.

Toilet Sign and lower practice internal wayfinding signs
Add toilet sign to assist wayfinding and lower internal signs
We are planning to add some nostalgic artwork to encourage reminiscing
Chair with arms
Add a chair with arms to support patients with addional needs
Add some plants in waiting area
Add some plants/greenery in waiting area to improve environment
Red toilet seat
Change toilet seat to a red one so it is easily distingushed from surroundings
