1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We will be undertaking the Dementia Friendly Care Home Toolkit to support our residents, their family and carers as best as we can.

Once we have completed this toolkit, we will receive a certificate stating that we have undertaken the toolkit and are Dementia Friendly.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

*Engaging with as many families as possible and inviting them to become Dementia Friendly.

*Getting all staff to become Dementia Friends.
