1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We offer all available treatment options so patients can make an informed decision.

We offer patient centred care.

We support all patients to live dental disease and pain free.

We offer written patient information leaflets, available in large font. We also provide written treatment plans.

We encourage and collect patient feedback and we discuss and act upon all reasonable feedback.

We offer a value service to our community.

Should we be invited to participate in research, we would be happy to contribute.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

As patients move towards later stage dementia, we would then consider refering them to specialised services.
