1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Hallmark Care Homes is a member of the Dementia Action Alliance, is signed up to the national Dementia Pledge and is actively involved in research projects to improve standards of dementia care.  Tammy Symmonds our Dementia Care Manager has over 14 years experience in the care industry and is a dementia champion.  She is keen to help as many local people as possible learn about dementia and what life is like living with this awful condition.  She would like to offer dementia friends sessions to all.
