1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Viaduct Care is commited to raising awareness of dementia, including through dementia champions workin in GP practices and in the community. We will support people to access local services through social prescribing, and develop support groups in partnership with GP practices.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

The challenge is to make the best use of limited resources so Viaduct Care will aim to work in an integrated way with partners to achieve the best outcomes.
