1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

As a home care company that offers dementia care we are in a good position within the community to deliver specialist dementia services to our clients. All are caregivers are  trained in dementia care and we offer City in Guilds accreredeted training .

We have a good relationship with local GP practices in the area and the Admiral Nurses. Were are also involved in supporting local charity groups on wirral:

Dementia Together Wirral - who offer activities and support for those affected by dementia and their carers. Role On -  a charity for former carers.DEEP group Wirral and Cheshire West - who we support and attended their monthly meetings.Wired Wirral - umbrella organisation to support disabled people and carers. We provide information at their Wirral Dementia Showcase  event. We offer free dementia workshops to community groups and local businesses. 

Our aim is to effecively signpost those affected with dementia to local services and groups available within Wirral.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

As a whole, we have not as yet encounted many barriers within our local community signposting services.

Our caregivers and office staff have a firm understanding of dementia care.
