1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We give all available treatment options so that the patient can make an informed decision. We offer patient centred treatment. We have leaflets and always given out patient's written treatment plans. As above we operate an honest and open door policy. We collect and act upon patient feedback where appropriate. We value all our patients opinions and appreciate their feedback. We are a valued part of the local community. We would embrace the opportunity to contribute to any research around Dementia and dental care.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

We understand that the population is getting older and the patients with Dementia may increase. As patient progress towards later stages we may have to refer onto specialist services.

2. Actions

  • Undertake Dementia Friends session every 2 years

    We are going to commit to completing the Dementia Friends session every 2 years and book via the Dementia Freinds website

    We are going to provide a ramp for patients to easier access the practice

  • We will make a sign for the reception stating patients can ask for water upon request

    We have made a sign for the waiting area stating if patients wish to have a drink of water to ask a member of staff

  • Toilet facility with wheelchair access

    We are being renovated and we are having a accessible toilet especially built.
