1. Action Plan
1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?
Part of our role in the local community of Noth Leed is to empower individuals to maintain their indepedance in thier own homes. We ensure all our clients take control of thier care whilst making decisions as to how their care should be provided. We provide outcome based objectives set out in the clients support plan to ensure our clients living with Dementia live fulfilled lives.
2. What are the challenges to delivering these outcomes from the perspective of your organisation?
At Tamar we feel the best way to overcome the challenges in delivering exceptional care to individuals living with Dementia starts with the training of out staff and changing the way we perceive individuals living with Dementia. With the right training provided this gives our staff the right tools to deliver the best possible care to our clients in the North Leeds community.
