1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We are a small company with many years  knowledge of living with dementia. We offer support to both the client and their carer/s.

We like to think we go above and beyond our job roles, we enable clients to remain living at home with their loved ones due to their attendance at The Swallows. We try at all times to promote independence and person centred care. Clients are able to attend Monday - Friday one or two days or even up to five times a week.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Every day is a new challenge with dementia as it affects eveyone it touches in different ways. Between the carer/s and ourselves working together we are maintaining a positive outcome resulting in the client being able to remain at home for as long as posible.
