We are a GP surgery based in Hornchurch. We have 3800 registered patients. We have 3 GP's and 1 Practice Nurse working at the surgery. We also have a Patient Participation Group.

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We have made sure that our staff members have completed Dementia Awareness Training. We also have a Dementia register, aling with a carers register. We offer longer appointments for patients with Dementia.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

We share the building with another GP surgery which can sometimes be confusing for patients. This also puts pressure on our appointment times, not always enabling us to see patients for long appoointments.
