1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We provide all our patients with all available treatment options so that that can make an informed choice.

We offer patient centred care.

We support patients to live pain and dental disease free.

We operate a transparent open door policy and have information leaflets available

We collect patient experience feedback and act upon suggestions and value comments

We provide a valuable service to the local community

If there was an oppurtunity to be involved in research around dental care and Dementia we aould be happy to contribute

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

As patients with Dementia move towards that later stages we may have to refer them on to specialist services.

We feel that patients with Dementia may need longer appointments and we can not always offer longer appointment slots, but we will facilitate where possible.

2. Actions

  • Fit 2 surgeries with privacy strips on the windows

    We have fitted privacy strips to 2 of our surgeries and removed the blinds in order to allow natural light into the surgery

  • Make sign in the waiting area with the practice name and address

    We are going to make a sign for the waiting area that states clearly the practice name and address

  • Provide coloured mouthwash for patients with visual impairment

    We have bought mouthwash tables for the  water that patients rinse with so that patients can locate the cup easier

  • Sign for direct patients to the upstairs surgeries

    We are going to make/buy a sign to place in the downstairs corridor to make it clearer for patients to make their way to the waiting area upstairs

  • We are going to order 'hot' and 'cold' stickers for the taps in the toilet

    We are going to place 'hot' and 'cold' stickers above the taps in the toilet to make it clearer for patients
