1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Action 1

We would like to raise awareness amongst our staff, patients and visitors regarding Dementia to ensure we are providing a safe and supportive environment. We will organise an awareness session for staff members

Action 2

We will make the practice dementia friendly with better signage etc.

Action 3

We will improve identification of carers of those with dementia to offer support.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

The only challenge we envisage is engagement.  We will do our utmost to offer support where it is required.
