1. Action Plan

We have a duty of care to all of our residents that live within the Care Home. We want to continue to  support our residents living with dementia in a holistic way and to include and support families through this journey.

We provide person-centred care planning, include and learn  from family and want to get involved with Community projects.

2. Actions

  • SERRY

    Staff - Identify a Dementia Champion, sign up to the Dementia Action Alliance, ensure staff include family with all aspects of care.

    Environment - encourage a Dementia Friendly environment to encourage residents to socialise and enjoy their home.

    Resident - All residents to have an up to date This is Me, this will travel with the resident if admitted to hospital and to include resident with family celebrations.

    Resources - utilise Dementia Community Support Team and Public Health England Resources

    Your Dementia Team- link up with local Dementia Support Teams to encourage joint working.
