1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

1. Ensure our community builder teams have opportunities to become Dementia Friends

2. Delivery of further dementia-related staff training as needed

3. Raise awareness about dementia and dementia services through social prescribing

4. Collaborate with the Exeter Alliance on a press piece/shared marketing

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

We find value in having carers / next of kin involved to help progress and avoid repetition and struggling to connect.
