    We will arrange for reception and admin staff to become ‘Dementia Friends’ in order to help increase understanding of dementia and how patients may be affected.

  • GP Information Packs

    Help ensure GPs and practice staff have up to date information about local services for people with dementia and their carers taking place at the Merton Dementia Hub and across the borough. Also ensure staff are aware of the referral process.

    Order up to 25 free copies of the Alzheimer’s Society ‘Dementia Guide’ for GPs and practice staff to give to patients with dementia and carers who may find it useful.

    Examine the automatic phone system and see if phone numbers associated with patients with dementia may be able to skip the automatic que and get straight through to a member of staff.

    Flag patients with dementia on the computer system to help ensure special care and consideration is taken when making contact or arranging appointments. This will especially help ensure that patents who miss appointments without prior arrangement can be followed up correctly.

    Encourage the use of ‘This is me’ (https://www.alzheimers.org.uk/get-support/publications-factsheets/this-is-me) – a record of patients individual needs and preferences, and reviews of their care by health professionals. Patients in the early stages of dementia could be encouraged to start getting in to the habit of carring their ‘This is me’ to appointments.

    We will use avenues such as the staff newsletter to ensure up to date information about local services for people with dementia and their carers, as well as tips on how to support patients with dementia, are sent out.
