Dementia Crisis Team North

Registered Mental Health nurses and occupational therapists that form a crisis team for people experiencing crisis following a diagnosis of dementia. once we have received a referral we have four hours to respond and assess a patient. We officer medical reviews, monitor medication changes, support and advice to families and sometimes we will need to sign post to OPMHT for social work allocation that can access respite beds and carry out social care assessments.

  • continue to support patients and families in the East Grinstead area

    Dementia Crisis North engage as much as possible with local initiatives

    sign post patients and families to relevant events, day centres and provide additional services of rapid response or setting up a package of care

    attend where possible any dementia friendly events to promote the service

  • Supporting carers to access & make aware of local places / resources that can offer continuing support following discharge from our service

    • Promote resources that are available to patients and their families for example: telelcare, carers support, just checking, GPS tracker.

    Highlight the council tax reduction once diagnosed, applying for attendance allowance if over 65, if under it would be PIP and services that offer grants for breaks away for carers

  • Advertise and promote who we are and how our service can be accessed

    • Support patients and families who experience crisis
    • Follow up on referrals we receive & carry out an assessment of needs
    • If required the patient can access our crisis bed which is based in Burgess Hill. This is for the family and patient to have some respite from their caring role
