St Barnabas Lincolnshire Hospice

Provide services to people living with life limiting illness. Through Day Therapy these include sessions on Breathlessness, anxiety, relaxation, fatigue management, mobility and exercise, diversional therapies and carers information and advanced care planning.

1. Action Plan

1. The National Dementia Declaration lists a number of outcomes that we are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Provide support to patients and carers from time of diagnosis through Day Therapy where they can be supported to make decisions about future care and work in partnership with other organisations to provide appropriate care during their illness and  including end of life care through our  Hospice at Home  service.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Ensuring robust triggers within disease specific pathways for referral to appropriate palliative care services. Developing partnerships to ensure shared care.

  • Improve the skills of the Dementia Care Workforce

    We will improve the skills of the dementia care workforce by developing and delivering high quality education programmes to staff across a range of dementia care settings and through the use of our approved trainer scheme.

  • Improve the awareness of the workforce about dementia care

    We are improving the understanding of the workforce about dementia care through an e-learning training programme. There is also on-going programme of information sessions for the Volunteers to become Dementia Friends.

  • Support people with early dementia to make Advanced Care Plans

    We are providing opportunities for people diagnosed with early dementia to attend our day therapy service and discuss and make advance care plans to support their future care

    We are in-reaching into a care home to deliver elements of day therapy to people with moderate to severe dementia who are unable to attend the day therapy service
