1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

All staff at Canvey Island's  Hook & Partners have undertaken a Dementia Friends session, we are supporting the new Dementia Navigator for the Island and will work with this service if our clients wish to gain their support.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

To get all staff together, we overcame this by undertaking the Dementia Friends session at our training day.

All staff were present.

2. Actions

    All staff have undertaken a Dementia Friends session.

    This was delivered by a member of the Dementia Community Support Team.

    We will be working with this team if our clients would like their support.
