1. Action Plan
1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?
We work with people in the community to build a package of care for them that suits them; giving them the option of what care the receive and how they receive it. We do this by ensuring the person is involved in all descisions from the start. We are also hoping to hold regular events in the community not just for our customers to give everyone a sense of belonging and community spirit.
2. What are the challenges to delivering these outcomes from the perspective of your organisation?
We have found it difficult to source specific dementia training and recently got in touch with our local dementia hub for support.
