1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Sortified CIC’s role is to operate at a local level to promote, listen, support and develop innovative approaches and tools to real social issues faced in local communities.  The organisation aims to influence, as well as to support through collaboration with affected groups and individuals.  Sortified CIC is a group of professionals from Commissioning and Provider health and social care backgrounds with experience in designing dementia services around needs, personalisation, choice & control and providing good quality information for people with dementia and their carers. The organisation has the ability to apply these skills and knowledge to support and influence local change and deliver projects with real benefits operating as an organisation and/or in conjunction with other Providers. 

 

 

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Sortified CIC is a developing organisation, so at an organisational level the challenge will be promoting the skills and knowledge the group has to offer to local communities, Commissioners and within DAAs. The needs of people affected by dementia do not have off the shelf solutions and Sortified is dedicated to being involved in developing and delivering real benefits that are person focused.

  • Joining local DAA and dementia groups

    To establish Sortified CIC’s resources and skills on dementia in the local area, and to join and participate in the development of local plans within DAA and dementia groups

  • Person centred approach

    We are applying for funding in collaboration with a local voluntary sector provider to develop a person centred tool in response to local dementia needs within identified care homes.

  • Raise Awareness

    Incorporate dementia into the organisation’s core work, ensuring all opportunities to raise awareness and understanding of dementia through development of other projects and membership on other forums are optimised.

    2016 - Fourth Quarter Update

    Completed. Several Dementia specific projects and programmes of activity have been developed and implemented. These include a walking project and development of a Dementia Friendly Bourne initiative.

    2015 - Third Quarter Update

    In progress. Several Dementia specific projects and programmes of activity are in development, or have moved to a funding application. Progress has moved at pace in recent weeks.

  • Local Awareness

    Sortified are working with local partners to help raise awareness of Dementia and work towards Dementia Friendly Community Status in Bourne, Lincolnshire.

    Part of this action will be supporting the rollout of Dementia Friends and training Sortified staff as Dementia Friends' Champions.

    2016 - Fourth Quarter Update

    We have worked with partners to develop a Dementia Friendly Bourne initiative. This will be going into its second year in 2017. Progress is slow and challenging but we have had buy in and received funding for further development

    2015 - Second Quarter Update

    This is a new action for Sortified. We have undertaken a local survey across Bourne, Lincolnshire, examining what would make the town more Dementia Friendly.

    We will examine the results and work with others to improve awareness locally.
