1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Your 5-Point Personalised Care Plan

Before you move in, we’ll talk to you, your doctors and your family about five key areas:

1.Personalising your bedroom suite

2.Your individualised care, nursing and support needs

3.Your personal interests, social life, former working life and companionship

4.Your favourite meals and dining choices

5.Keeping your family and friends in touch with you, visits and activities

The result:

•Care that is as individual as you are

•Care that changes as your needs change

•Care delivered by expert, trained, experienced carers and support staff

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

No major challenges as well trained staff and team members. Support for families could help us.
