1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

• We aim to promote the necessity and importance of planning for the future with Lasting Power of Attorney & Wills. Sequacity aims to give guidance and support to clients & their family members to make sure they have all the facts needed to set up LPA’s & Wills. • Highlighting to clients the importance of having an up to date Will and the consequences of dying without a Will (Intestate) • Making the process of Drafting Wills and setting up Lasting Power of Attorney’s understandable and putting clients at ease working with them in the comfort of their own homes giving a very personal and professional service. • Offering Clients Free Consultations in their own home, this could be one or more consultation depending on the client’s circumstances. • Ensure staff have a high level of awareness and opportunities to undertake training regarding capacity • All Staff to become Dementia Friends

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

• Personal wealth of clients and the affordability of having a Will Written or Lasting Power of Attorney (LPA’s) put in place. Advising clients on low income on how the costs could potentially be reduced to set up LPA’s • Raising awareness of the impact of not having a Lasting Power of Attorney in place it is not always understood until something happens and its too late to put in place • What happens when you die intestate a high percentage of clients do not have this information • Making people aware that they need to get all their Legal affairs in order before they lose the Mental Capacity to do so. When people are healthy they don’t think that this is important so don’t see the value in creating the documents required to secure their futures
