1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We give all of our patients the time to discuss and understand the treatment they need. We also offer extra appointments with one of our nurse for patients to ask any questions they may have and have treatments explained in different ways  so they are able to understand and decide what treatment they would like. We also have the option of our patients to bring someone along with them to offer the patient support. All treatments have writing quotes and information leaflets.

On top of this, everyone within the practice is trained in safe guarding vulnerable adults so are aware of what to do if they feel a patient is in danger. We have a safe guarding lead and training is up dated annually.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

The only challenge I can see would be if the business was to grow so big we wouldn't have the time or space to give patients as much time as they need. If this was to happen, we would extend our opening hours and/or move buildings and employ more staff.
