1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

When I coordinate with a family member or the gaurdian I always ensure they are aware of the homes restrictions and provide advice on what items are of importance, so those with memorys, pictures, photographs, furniture etc. I also help people understand a little better with what they may be experiencing, so if it's all to overwhelming I can assist in calming them and their relative by understanding and a little patiance.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

The only challenge is time and providing I'm aware of the situation up front then I always allow time for unplanned eventualities.
