1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

PlumCare understands that every individual have personal choice and the right to have control or influence over decisions about them

PlumCare services are designed around our customers and their specific needs

PlumCare supports believes that everyone has the right to live their life the way they want to

PlumCare understands that everyone is assumed to have knowledge and know how to get what they need. 

PlumCare understands the importance of every individual's right to live in an enabling environment where they feel valued and understood

PlumCare believes inthe importance of having a sense of belonging and of being a valued part of family, community and civic life

PlumCare will keep upto date withDementia related research going on which delivers a better life for everyone now and gives hope for the future

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

PlumCare provides comprehensive Dementia awareness training to all staff.  This is followed up with regular staff supervisions and coaching to ensure the needs of all our customers and staff are met.
