We have committed to provide and promote individual and community based activities for people on the dementia pathway and their carers. Our Forget Me Not weekly café is key in leading the support and signposting that we offer. Thus is a resource that can be used by the wider community and beyond. We are happy for our practise to be shared further afield in order to help other communities develop the provision they offer.

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We have a plan to raise awareness about dementia in key organisations and businesses within the community.

We work towards developing a strong voice for people with dementia living in our community.

We offer a weekly Forget Me Not cafe where regularly we have upwards of 30 attending (those on demenetia pathway and their carers) plus a Forget Me Not Church service once a month which they all attend regularly.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

We recognise that there are times when those on the dementia pathway can be considered to be a "vulnerable adult" and therefore we have a safeguarding policy and all volunteers on our team undergo regular Safeguarding training and are all DBS checked.

Transport can occasionally be an issue as we cover a rural area and many of the people who attend Forget Me Not are wheelchair users - we can seeking funding to purchase a wheelchair access vehicle which should help to solve these issues.

Getting the local busines community on board is a long process - however, we will continue to do all we can to raise awareness of dementia both within the church community and further afield - through hosting training opportunities, information evenings and have available information packs.

We continue to seek ways in which to support other organisations within the community to become more dementia friendly: the parish council, local businesses through the Ingenuity in Business Group and the school.
