1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We have ensured that all our staff including GPs, Nurses and non-clinical staff have all received dementia awareness training.

we have a register of patients diagnosed with dementia

Our PPG have successfully set up a dementia carers cafe and they have successfully been part of dementia awareness week with support from the practice.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Identifing patients with dementia and ensuring the services meet their requirements effeciently


2. Actions

  • Dementia Awareness for all Staff

    All staff have reiceved a Dementia Friends session and have undertaken dementia awareness training.

  • PPG awareness

    Our patient participation group (PPG) received a talk from Dementia Friends and have successfully set up a dementia carers cafe.

  • Dementia Awareness Week

    Our PPG and staff (including GPs) took part in Dementia Awareness week. We had a stall outside our surgery manned by our PPG and staff who successfully raised awareness to our patients about support available within the community.
  • Signage in the practice for patients with dementia

    This year we plan to look into the cost of signage and the environment within the practice for patients with dementia
