Triangle Community Services (Home Care Malvern) offers the support people need to live well and stay in their own home. Founded on the experience and heritage of Friends of the Elderly we give people the opportunity to live healthy, happy lives for as long as possible.

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

            

We help people live independently with visits from a length of 30 minutes through to high dependency packages.

We understand that everyone is different and create a personalised Care and Support Plan.

 

Daily Living Support and Companionship: We can provide help personal care, with preparing meals, shopping, household chores, accompanying to medical appointments and social activities.

Support for Family and Carers:  we can help with short and long term breaks or holidays.

Complex Care: For those living with dementia our ‘Dementia Awareness’ trained team provide a high level of support built around individual needs. We work in partnership with health care teams.

Readjustment: We can help people build their confidence and strengthen their coping skills.

 

2. What are the challenges to delivering these outcomes from the perspective of your organisation?


  • Raising awareness amongst all our staff, keeping training ongoing and up-to-date.
  • Clients’ home environments- only able to suggest simple appropriate changes

  • Join our local Dementia Action Alliance

    To work together on a local level with other providers and organisations to raise public awareness of dementia and promote the services available to support people

    Our home care service aims to provide the best, person –centred care for people in their own homes; to enable people with dementia to feel included in their community, be more independent and have more choice and control
