1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

The Practice delivers sensitive medical care to patients.  We support people to make informed choices on their healthcare and choose the options that suit them best.  We have a role in supporting patients, their families and carers.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

People can be very frightened to receive a diagnosis of dementia and can avoid seeking help at the onset of possible symptoms.  We seek to find ways of improving our diagnosis rates, improving our staff and patient awareness and improving our physical surroundings to support people affected with memory problems when they visit the practice.

2. Actions

  • Improving signage

    We will be installing dementia friendly signage around the surgery to help people navigate around the building.

  • Education

    We will be running an education session for our staff, held by a registered Dementia Champion to raise awareness of the staff.

  • Community Engagement

    We will be running a community event in the style of a coffee morning to raise awareness of dementia and encourage people who have early symptoms to seek further help and support.  Registered Dementia Champion will  facilitate this highly interactive session.
