1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We have a duty of care to all of our patients within the surgery. We want to continue to diagnose and support people living with dementia in a timely way and to help to avoid times of crisis through robust care planning, information, educationand support.

 

Prevent well – we continue to promote Healthy living.

Diagnose well – diagnose in a timely manner, include yearly checks and reviews.

Support well – to work with outside agencies that provide extra support.

Living well – to encourage independent living and to be active in the community.

Dying well – to actively encourage early participation in End of life plans and patient centred care planning.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

To implement iSPACE across the practice and involve all staff, PPG and public.

2. Actions

  • ISPACE

    dentify: One or two Dementia Champions within the practice and to join the SDAA.Staff: for all staff to undertake a Dementia Friends Session, and to wear the badge.

    Staff: that are skilled and have time to care.

    Partnership: Working with carers, family and friends.

    Assessment: early identification of Dementia – encourage a positive image of Dementia.

    Care Plans: encourage patient centre plans that are filled in early after diagnosis.

    Environments: that are Dementia Friendly.
