Southend Therapy And Recovery Team

START is goal based and work alongside patients to assess their capabilities in line with Therapist assessments. They are a reablement and rehab team.

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

All patients with Dementia received a person centred care. If they do not have capacity then their NOK and any carers are involved in any decision making.

All daily living skills are assessed within their own home- taking into account their previous preferences that their families have advised on during the assessment period.


Each assessment by Southend Therapy and Recovery Team is tailored to the individual preferences and all staff work alongside the patient observing their abilities and ensuring that they are as least intrusive as possible to keep them safe.

All staff in START are trained to ensure that a holistic assessment is included, any issues that arise in not within START responsibility - START staff will signpost to other external providers and agencies.

Regardless of peoples diseases and conditions -START staff ensure that the patient is involved in all decision making. Our motto above the door is "everything that we do is in the best interest of our patients."

START staff will, in conjunction with family ensuring that past preferences are recognised, will work with families to ensure community inclusion is included within their assessment. START staff have knowledge of day centres and other community resources that they can signpost to.

START staff are always looking at aids and adaptations and new ways of working with adults with a diagnosis of dementia to see if there is any technology that would assist. The dementia champion regularly looks at research and new ways of working with adults with this diagnosis,

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

We have lots of staff that work different shifts within the 24 hour perioud.

2. Actions

  • Staff to continue to undertake Dementia Friends Sessions.

    The team has a Dementia Friends Champion and will continue to hold Dementia Friends sessions for new members of staff.
