1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We provide meaningful lifestyles and we ensure that our residents are involved in decision making. Residents decide on activities within the home, entertainers, arts and crafts or trips out. We also invite local dementia cafes and community groups to use our facilities, such as the cinema and coffee shop. We host regular events that are opened up to the whole community. We ensure that residents, relatives and staff feel valued and supported (regular surveys asking for feedback, resident, relative and staff meetings).

We have incentives for staff such as Care UK perks, Gem awards for going the extra mile (can be nominated by anyone).

Residents have access to resources such as wifi, telephone, sky television, postal service, mobile library. Each resident has a care plan tailored to their individual needs and preferences. Each resident has a life story book – documents the residents life, family, work life, family tree, likes/dislikes, special dates etc.

We ensure that staff have access to training and we also host informative talks for relatives and the community such as “understanding dementia” and “falls prevention and awareness”.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

We are learning and adapting all the time. You cannot have a one size fits all approach. Lack of resources in the community. Changing the stigma of dementia.

2. Actions

  • We will help to make the community more dementia friendly

We will help to stop the negative stigma around the diagnosis of dementia and we will help to make the community more dementia friendly. We will ensure that all members of staff are dementia friends.
