1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

 TLC Care Services LLP understands that  people living with dementia are not all the same, so for every person with dementia that we support, we  ensure this by taking a holistic approach to ensure that their needs and wishes are fulfilled through the packages that we provide.  Our Carers are trained to hone in on their listening skills, to always maintain empathy and to expect that no day is ever the same for a person with dementia and we encourage them to exercise flexibility as they support the service user (s).

To ensure best practice, all our carers support our service users with dignity and respect especially surrounding issues when assisting them with personal hygiene or when service users prefer to do things on their own even if they are doing them slowly. Our Carers are patient and very supportive as they assist the service user.

TLC Care Services LLP promotes positive communication and feedback;  We encourage communication between all staff and the service users these promotes information sharing within the organisation which not only helps us to improve our services to people with dementia but also ensure the care we provide them is up to date and relevant to their needs

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Some of the Challenges to delivering these outcomes are;

Some of the service users with dementia have issues with memory &  language problems; its sometimes challenging for them to grasp issues around their care in real time & to understand these issues.  TLC Care Services LLP however tries to ensure that we work with the service user, their family or next of kin and any supporting organisations to ensure that our service users understand how we are helping them and how they can communicate with us in case they have any concerns or queries.

There needs to be a wider training curriculum on care for people with dementia.  As Dementia manifests itself differently in every person, though our carers are trained on dementia care, some of this knowledge is not very practical in reality.  There is a need for more reality based training of how to best support people with dementia.

  • Our Way Forward

    -TLC Care Services LLP aims to ensure effective care management systems are in place that will help us better support people with Dementia. We will work with other auxiliary services such as the GP, Community Practitioner Nurses, Nutritionists, Speech and Language Therapists, the Local Authority and any other agencies to ensure that our service users have access to services that ensure their health and wellbeing. -TLC Care Services LLP will continue to carry out follow up calls, courtesy calls, home visits and the use of questionnaires, to ensure that we are in daily communication with our service users and to help us keep up to date on how we can best support them. -TLC Care Services LLP will scout for more enlightening training opportunities for our staff as they support people with Dementia, this we hope will equip our staff with professional development,& better ways of working so that they carry out their care for our service users efficiently, effectively there by promoting the service users safety & independence while maintaining their dignity. -TLC Care Services LLP aims to implement assistive technology such as lone care worker alarms to ensure that our carers are safe as they support our service users. In addition, we have a system in place that allows carers to send us updates or any concerns around the clients they support in real time and these are/will be acted on immediately. -TLC Care Services LLP recognises that our carers are at the fore front of assisting people with dementia. We aim at coming up with incentives and ways of encouraging our carers by putting up; awards for 'Carers of The Month,' encouraging them and ensuring feedback for any compliments from the service user and always having open discussions and supervisions with them for any concerns and or queries they might have.
