Gracewell of Kentford

Gracewell of Kentford is a care home for the elderly. We cater for residents with nursing, residential and memory care requirements. Our vision is to help to make Newmarket dementia friendly and to tackle the stigma associated with dementia. We are looking to achieve this though education, awareness and open communication.

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Gracewell of Kentford can make a difference by continuing to serve the residents and the families who have chosen to live within our care home. In addition we will raise awareness of dementia and how to support those living with dementia in our community by delivering free information sessions within our home and in the local community. We will also highlight the importance of the NDAA as every appropriate opportunity.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

A challenge may be time and diary restrictions due to other work and community commitments.

To address this challenge by advance diary planning and time managing, remember the importance of the NDAA.
