Crown Medical Practice

We are a 3 doctor GP Surgery based at Tamworth Health Centre, in the town centre. We have just under 4300 patients. One of the GP's, Practice Manager and a member of our Patient Participation Group have recently had Dementia Friends training at Marmion House, Tamworth.

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We will help create greater awareness.

Get a list of support services for patients and their families

Our staff  will be more aware of how best to deal with dementia patients and communicate better with them.

Introdude more effective and easy to understand signage.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

We are a busy GP surgery with lots of demands on our time.

We don't own the building so this limits what changes we can make.

A lot of our dementia patients are housebound and there are limited resources and care for them.

  • Train all staff to be Dementia Friends

    Trying to get all staff trained to be Dementia Friends.

  • Participate in Dementia Awareness Week

    A member of our Patient Participation Group is planning to have a stall in our surgery during this week and actively promote dementia. She is planning to sell twiddle muffs and donate what she gets to the cause.

  • Facilitate Early Diagnosis

    As a practice we try and facilitate early diagnosis by trying to spot patients that might be developing increased memory loss and then referring them to the memory loss clinic.
