At Home Care Solutions, our focus is to make a positive difference to people’s lives in Bedfordshire, Buckinghamshire, Cambridgeshire, Hertfordshire, Lincolnshire, Luton, Northamptonshire, Nottinghamshire and Oxfordshire by sustaining independence. Home Care Solutions provides home care services to elderly people and to people who need help and support at home because of illness or disability. Our flexible care and support is tailored to individual needs. We can provide as little or as much assistance as is required, including specialist care services such as dementia care and palliative care, 24 hours a day, 365 days a year. 1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

At Home Care Solutions our focus is to make a positive difference to individual’s lives to help them live well at home with dementia.

By working with the Department of Health’s policy ‘Improving Care for People with Dementia’’ our person centred care plans help each individual and their carer/families to achieve their desired outcomes.

All of our branch managers and senior managers are Dignity Champions and we promote to all carers the opportunity to sign up to become Dignity in Care Champions.

Being Accredited as Investors in People we continue to develop and train our workforce to allow them the most latest information, skills and knowledge on Dementia, ensuring that they can provide the best level of dementia care at home to our customers.

As we support dementia suffers day-in day-out, we know exactly how the disease affects people and their families, which is why we  actively participate in Dementia Awareness Week which is the Alzheimer’s Society’s annual flagship awareness-raising week. This along with Dementia Awareness Days helps us in creating awareness in the local community. By supporting these events, our aim is to get people talking about the disease, raising money for the Alzheimer’s Society at the same time.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

A major challenge remains highlighting the awareness of Dementia both in the community, with our carers and also with the families we indirectly support through our Domiciliary Care Services to individuals with their own homes.

2. Actions

  • Improve the Awareness of Dementia in our workforce and our local communities

    We will improve the knowledge and understanding of Dementia by participating in annual Dementia Awareness Weeks and creating local events in support of these weeks through our branch managers by the end of 2013.
