1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

By referring and/or sign-posting our patients to the correct help and suppport which they need. 

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Not knowing what services are available to our patients.

  • Action Plan : Better Outcomes for People with Dementia

    Researching ways to deliver better outcomes for people with dementia and their carers.

  • Support for people with dementia and their carers

    We will look for ways to better support those people with dementia and their carers.

  • Referrals for people with dementia

    We will look to refer people with dementia to centres which can best provide them with the support and care which they need.
