1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

With an ever increasingly older generation patient base we feel it is better to have a good understanding of dementia and make ALL of our patients experiences a good one. We will achieve this with regular training and suggestions on how to make the practice more dementia patient friendly. We will start by putting the above actions into place. Once achieved we will then put another 3 in place to achieve.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Cost is always a factor and the larger changes will need to be prioritized. As we are short on space a quiet room maybe hard to find.

2. Actions

  • Be mindful when replacing flooring

    What we are going to do: Order a light flooring to replace the current dark carpet. Replace dark mats with lighter versions.

    All team members are to sign up to the Dementia Friends Champion. Before June 2018.

  • Calming Environment

    To introduce sessions that have a calmer environment. Within school hours and making it a adult only environment. Remove the toys from the children’s area and just provide books to have a quieter environment at all times.
