1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Uttoxeter Cares is a community development project focused around a community centre. It was founded to start a conversation with our local community around issues to do with later and end of life care . The centre offers support and education to families living with dementia. We provide events to showcase the support available by other organisations as well as our own.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

The awareness of our project locally is fairly low (we are a new project). Local people's perceptions of dementia has been identified as a barrier to diagnosis and people seeking/accessing support.
