1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Action for Carers Surrey is led by carers, run by an executive committee of carers and supported by professionals from Health, Social Services and the voluntary sector.
Our main aim is to raise awareness of carers’ needs and concerns throughout the county and to work in partnership with statutory services such as health care and social care teams to promote how best they can serve carers within Surrey. This includes GPs and Primary Care professionals.
