Moatfield Surgery
GP Surgery Moatfield Surgery would like to become a member of the DDA to help support existing projects in the community, as well as be involved in new projects. Patients in high risk groups are reviewed and given a memory test if appropriate to identify dementia early. Patients and their carers can then be offered the help and support required. In addition to this the practice will have a suggestions box in the practice and there will also be a survey sent to those families living with dementia asking what is important to them.

The practice will follow up any actions implemented by discussing with patients, carers and staff at regular intervals and asking for their feedback.

create a welcoming environment
Review the layout of our practice to remove/ reduce hazards/ concerns for those living with dementia. This will include looking at signage, the entrance to the practice, access to toilets and consulting rooms etc.

Dementia Awareness for Staff
Arrange Dementia friends training for reception, admin staff and members of the PPG. Promote Dementia Awareness Week from 21st-27th May 2018, inviting Dementia Friends Volunteers to have a small stand in reception promoting Dementia Friends. Have a raffle for the Alzheimer’s Society.

Update Carer Information
Contact all patients on the dementia register and those patients identified as caring for someone with dementia to update their information. Carers will be supported in two ways: 1) how we can work with them to improve the service we offer to the person they care for, eg allow longer appointments, later in the date appointments etc. and;
2) how we can support them as carers , ensuring they have regular health checks and offered flu vaccines etc.
Carers will be advised of the Care Coordinators role and how they can help with signposting etc. All referral letters from the surgery will include information and contact details of the carer.
If patients do not have a carer, then an alert will be placed on the patients records to prompt/remind them of any appointments.
