Abbey Medical practice was founded on 1st October 1997 with the guiding principle of providing the best possible quality of service to patients within the total resources at the practice's disposal. As a partnership of four GPs, we have established a solid commitment to high quality clinical care which is in step with current best practice, but which retains the values of traditional General practice.

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Being a DAA member will allow Abbey Medical Practice to have a better understanding with other organisations and to advise and support patients to make a real difference to anyone affected by dementia.

Having recently appointed a Memory Loss Advisor to the surgery allows a key point of contact to provide appropriate and tailored information and support for people affected by dementia.

  • Create dementia awareness stand/area

    Dedicate areas/stands for both staff in staffroom and patients in the waiting room to access literature  and information about local available services.

    Ensure all staff are trained to become Dementia Friends by March 2018. Improve awareness on our  website and enable patients to access screening to establish early diagnosis if they are worried about their memory.

  • Improve diagnosis rates

    Ensure patients are coded correctly and included on the dementia register and allow patients access to our Memory Loss Advisor who can ensure a key point of contact to provide appropriate and tailored information and support from early assessment to point of diagnosis.
