We are a GP Practice in a Primary Care Setting, offering our patient population of 11,213 appointments along with medical advice by GP’s/Advanced Nurse Practitioners/Practice Nurses/Pharmacists amongst other attached Community Teams either with same day access or future routine appointments in core hours (Monday-Friday). Our Receptionist and Administrative staff, take multiple calls off patients with queries along with greeting and signposting them face to face on arrival at the Practice. We support our patients with preventative health/self-care management whether diagnosed with a Chronic Disease, Medicine Management or General Medical Advice. Primary Care is an ever increasing demand on health resources during core hours.

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We would like to join the local DAA as we think this will be an important step towards becoming dementia-friendly. In order to establish what actions to take we will discuss this with PPG members (Patient Participation Group) and listen to patients views both personal and what they are made aware of in the media.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Time to initiate contacts for promotional materials then discussing within our Primary Care Team.

  • Flexible appointment times

    As a General Practice within Northfield Village we have a 59 bed Dementia Unit adjacent to the Practice where residents register with the Practice on admission/arrival, appropriate patients are asked to attend the Practice to be reviewed by a GP/Health Care Professional, these appointments are used at the discretion of reception staff and adhere to flexibility at all times.

  • Locality Memory Hub

    Stafford Health & Wellbeing is the Hub for “Stafford Primary Care Alliance” which includes Gnosall Health Centre, Wolverhampton Road Surgery, Castlefields Surgery and Holmcroft Surgery and will be identifying a staff member to become a Dementia Champion in order to take responsibility of Dementia within the Practice & Locality.  All staff are dementia friends and wear a badge to alert patients/carers to this affect.

    As a pilot GP Practice for this commissioned software by our Local CCG, this has now been successfully rolled out to Stafford & Surrounds.  West Midlands Ambulance Service are responsible for updating this to enable reception staff to navigate patients accordingly.  Navigation materials with voluntary sector services to support carers/patients alike in their journey.
